INCIDENT/OCCURRENCE REPORT IN ACCORDANCE WITH EU REGULATION 376/2014 (AS AMENDED BY 2018/1139) FOR CONTINUING AIRWORTHINESS & TRAINING ORGANISATIONS:

– ATO ULTRAMAGIC (ORA.GEN.160)     
– CAO ULTRAMAGIC (ML.A.202)

With the aim of analyze the incident you are reporting, we would need, if possible, the following data: balloon registration, type of flight (school or passengers transportation), phase of flight (takeoff, cruise, landing), place, date and hour, role (pilot, ground support staff, client, instructor, student…). This report will be anonymous, but If you want, you can leave your personal data and e-mail in order to receive an answer from us.
